• Referred by an approved partner (such as a senior advisor, discharge planner, or healthcare professional)

Step-by-Step Process

1. Apply
   Complete the Golden Gap Application Form.
2. Submit Documentation
   Provide income verification and any required identification documents.
3. Medicaid Case Review
   A case manager reviews documentation to confirm likely eligibility for the New Choices Waiver.
4. Approval & Reserve Funding
   Once approved, funds are earmarked to support the candidate.
5. Select Assisted Living Community
   The senior selects a community that accepts New Choices Waiver participants.
6. Monthly Support Payments
   Golden Gap pays the monthly difference between the assisted living cost and the senior’s income directly to the community.
7. Medicaid Transition
   After approximately 12 months in assisted living, the New Choices Waiver application is submitted. When approved, Medicaid begins and Golden Gap support ends.
